    Intense or widespread
    2+ to 3+ or >1.0 - 3.5 g/24 hrs
    Symptomatic but no hydronephrosis, sepsis or renal dysfunction; dilation or endoscopic repair or stent placement indicated
    Symptomatic, associated with altered renal/GU function
