An event in the personal medical history of heart failure, a condition in which the heart is unable to pump enough blood to meet the body's requirements. (NCI Thesaurus)
Definition 2
Physician documentation or report of any of the following symptoms of heart failure prior to this care encounter described as unusual dyspnea on light exertion, recurrent dyspnea occurring in the supine position, fluid retention, low cardiac output secondary to cardiac dysfunction; or the description of rales, jugular venous distension, or pulmonary edema. A previous hospital admission with principal diagnosis of heart failure is considered evidence of heart failure history. (NCI Thesaurus)
