A standardized survey developed and published by Kurt Kroenke, Robert Spitzer and Janet Williams in 2002 in the journal Psychosomatic Medicine which is used to assess the severity of an individual's somatic symptoms. This self-administered instrument contains 15 items pertaining to topics such as stomach pain, headaches, dizziness, and trouble sleeping. Each item is rated on a scale of 0-2 with 0 being 'not bothered at all' and 2 being 'bothered a lot.' (NCI Thesaurus)
