A 36 question survey for measuring health status and outcomes from the patient's point of view. Designed for use in surveys of general and specific populations, health policy evaluations, and clinical practice and research. Measures 8 health concepts: 1) Limitations in physical activities because of health problems, 2) Limitations in usual role activities because of physical health problems, 3) Bodily pain, 4) General health perceptions, 5) Vitality (energy and fatigue), 6) Limitations in social activities because of physical or emotional problems, 7) Limitations in usual role activities because of emotional problems, 8) Mental health (psychological distress and well-being).
